Menopause is a natural event which normally occurs between the ages of 45 and 55, beginning, on average, at age 51.

During menopause, ovulation (egg production) stops and menstruation becomes less frequent, eventually stopping altogether. Once menopause is complete (called postmenopause), you can no longer become pregnant.

The symptoms of menopause are caused by changes in estrogen and progesterone levels. As the ovaries become less functional, they produce less of these hormones and the body responds accordingly. The specific symptoms you experience and how significant (mild, moderate, or severe) varies from woman to woman.

In some women, menstrual flow comes to a sudden halt. More commonly, however, it tapers off, both in amount and duration of flow. During this time, often called perimenopause, your menstrual periods generally become either more closely or more widely spaced. This irregularity may last for 1 to 3 years before menstruation finally ends completely.

A gradual decrease of estrogen generally allows your body to slowly adjust to the hormonal changes. When estrogen drops suddenly, as is seen when the ovaries are removed surgically (called surgical menopause), symptoms can be even more severe.

• A woman's eggs become less active. Normally, FSH, or follicle-stimulating hormone (a reproductive hormone), is the substance responsible for the growth of ovarian follicles (eggs) during the first half of a woman’s menstrual cycle. The active growth of the eggs is the stimulating signal that induces the ovaries to produce estrogen and progesterone. As a woman ages, her eggs become less active and more resistant to FSH, triggering little or no signal to induce the ovaries. Thus, the ovaries dramatically reduce their production of estrogen and progesterone.

• A woman's ovaries become less functional. Similar to the eggs, aging also leads to less functional ovaries. The sluggish ovaries can not effectively produce estrogen and progesterone even when they are induced.

The combined result of these two direct causes is a severe drop in the estrogen and progesterone level in the body — which is responsible for all the symptoms of menopause/ perimenopause — because these hormones affect many parts of the body, including the blood vessels, heart, bone, breasts, uterus, urinary system, skin, and brain.

• Hot flashes and skin flushing. Hot flashes are the most common symptom of menopause. According to some studies, hot flashes occur in as many as 75% of perimenopausal women. Hot flash symptoms vary among women. Commonly, the hot flash may begin with a feeling of nausea or a headache, followed by a wave of heat, sweats, flushed skin, and palpitations (feeling a strong heartbeat). Hot flashes often increase skin temperature and pulse, and they often cause night sweats and insomnia (sleeplessness).

• Bone loss: Rapid bone loss is common during the perimenopausal years. Most women reach their peak bone density when they are 25-30 years of age. After that, bone loss averages 0.13% per year. During perimenopause, bone loss accelerates to about a 3% loss per year. Later, it drops off to about a 2% loss per year. No pain is usually associated with bone loss. However, bone loss can cause osteoporosis, a condition that increases the risk of bone fractures. These fractures can be intensely painful and can interfere with daily life. They also can increase the risk of death.

• Cholesterol: Cholesterol profiles also change significantly at the time of menopause/ perimenopause. Total cholesterol and LDL (bad) cholesterol increase. Increased LDL cholesterol is associated with an increased risk of heart disease.
