Order By Fax

To order by fax:

Step #1. Select your shipping location (at the bottom of this page) to access our secure online Shopping Cart.

Step #2. Follow the instructions on our Shopping Cart to generate an order form.

Step #3. Print or hand write this order form and fill in the payment table at the bottom of the form as follows:

• Payment By: Your Credit Card or Debit Card Type

• CC Number: Your Card Number and Verification Number

• Expiration: Your Card Expiration Date

• Signature: Your Hand Written Signature

Step #4. Fax the order form to 212-591-6810.
